ABSTRACT

The prevalence rate of nonalcoholic fatty liver disease (NAFLD) has been increasing year by year.However,since the complex pathogenesis of NAFLD has not been fully clarified,there still lacks a gold standard for the diagnosis and treatment of NAFLD.5-Hydroxytryptamine (5-HT) can affect the development and progression of NAFLD via several links and thus change the course of NAFLD.This article introduces the association between 5-HT and NAFLD,the role of 5-HT in the pathogenesis of NAFLD,and its future application in treatment.

ABSTRACT

Objective To investigate the relationship between nonalcoholic fatty liver disease and the severity of acute pancreatitis,the data of patients with acute pancreatitis was collected to provide clinic basis for the diagnosis and treatment,severity assessment,prognostic judgement,and prevention of acute pancreatitis. Methods SPSS22.0 was used to analyze all data.Mann-Whitney U test(in case of non-normal distribution)was used to compare data between two groups.Chi-square test was used to counting data.Risk factors were analyzed by using logistic regression analysis. Results The severity of acute pancreatitis in patients with nonalcoholic fatty liver disease had a statistically significance with that of patients without nonalcoholic fatty liver disease(χ2=3.968, P = 0.046);nonalcoholic fatty liver disease(95% CI:4.132 ~ 22.575,OR = 9.658,P < 0.01)had a correlation with the severity of acute pancreatitis.Conclusion Nonalcoholic fatty liver disease was associated with the severity of acute pancreatitis.Nonalcoholic fatty liver disease was the risk factors for the severity of acute pancreatitis.

ABSTRACT

This study employed a Bac-to-Bac/Bombyx mori bioreactor to mass-produce immunogenic urease subunit B (UreB) from Helicobacter pylori. The signal peptide bombyxin from B. mori was used to promote secretory expression to improve expression levels and was designed and integrated into the UreB gene to generate the Bacmid/BmNPV/(signal peptide)-UreB baculovirus expression system. To determine whether the bombyxin signal peptide resulted in secretory expression of recombinant UreB (rUreB) and to determine the secretory efficiency, we tested the secretory expression level of rUreB in Bm5 cells using ELISA. To further investigate whether secretory expression affected cell viability, cells were evaluated using 0.4% trypan blue staining, and Bacmid/BmNPV/UreB without the signal peptide served as a control. The above recombinant bacmid constructs were injected to silkworm larvae, and the secretory expression level of rUreB was detected using SDS-PAGE and semi-quantitative western blot analysis. The results indicated that the bombyxin signal peptide directed the secretory expression of rUreB and that this expression improved the viability of Bm5 cells. Moreover, the results showed that the expression level of rUreB was 1.5 times higher with the Bacmid/BmNPV constructs containing the bombyxin signal sequence than those without the signal sequence. These results demonstrate that secretory expression can enhance rUreB expression levels and is likely to aid in the large-scale expression and yield of rUreB in silkworm larvae.

ABSTRACT

Objective To present our experience in two-stage procedure immediate breast reconstruction,which aimed to retain local tissue condition for breast reconstruction,overcome the limitation of radiotherapy on implant reconstruction,and simplify the procedure as well as improve cosmetic result.Methods The proceduces of our method were divided into two stages:in the first stage,the round-shaped expander was implanted in the subpectoral major space during the procedure of mastectomy; at the same time as expander implanted,the first saline injection was performed; with 4 to 8 weeks of inflation,the tissue expander offered adequate tissue for breast reconstruction; in the second stage,silicon gel implant,latissimus dorsi muscle flap,extended latissimus dorsi muscle flap and deep inferior epigastic perforatior flap were used for breast reconstruction after the tissue expander was exchanged.Results Thirty-four patients had undergone the two-staged breast reconstruction using tissue expansion.The average time of therapy was 5.5 months.There were no postoperative complications such as implant exposure,additional scar,and flap necrosis.The follow-up time was 6 to 18 months and the result showed excellent contour of the breast at a satisfactory rate of 97.1%.Conclusions Two-staged technique using tissue expansion in breast reconstruction is easily done and the complications are rare.The technique possesses advantages such as avoiding affection of radiotherapy on silicongel implant and elasticity of skin,retaining maximal local tissue for breast reconstruction.There is no necessary for correction of skin defect and additional scarring,thus the patch-like appearance of breast is avoided.The patients need not to experience depression post mastectomy.Two-staged procedure immediate breast reconstruction is a safe and reliable technique that is especially applicable to the patients who need radiotherapy.

ABSTRACT

<p><b>OBJECTIVE</b>To discuss the feasibility of the operation of minimal invasive with gallbladder preserved via choledochoscopy.</p><p><b>METHODS</b>From February 1992 to June 2006, there were 760 patients who underwent cholecystolithiasis treated with the minimal invasive operation with gallbladder preserved via choledochoscopy, among which there were 428 males and 332 females, aged from 18 to 81 years old. All cases were diagnosed by ultrasonography and their gallbladder functions were proved normal by the examination of oral cholecystography or ECT before operation. In the operation gallstones were removed from gallbladder completely.</p><p><b>RESULTS</b>There were 612 cases who were followed up for 1-15 years and the follow-up rate was 80.5%. All patients recovered well after operation. The post-operation rate of recurrence of gallstone was 0.49%, 4.39%, 5.83%, 6.60%, 7.21% and 8.38% within the first year, the second year, the third year, the fifth year, the seventh year and the ninth year respectively, rate of recurrence of gallstone were 10.11% within both the tenth and the fifteenth year.</p><p><b>CONCLUSIONS</b>The minimal invasive operation with gallbladder preserved via choledochoscopy is effective to cholecystolithiasis patients whose gallbladder function is normal. It is a feasible operation that preserves the normal functional gallbladder and improves the patients' life quality.</p>

ABSTRACT

Shanghanlun contains the theory of stagnation,though which is concealed among the treating system.This theory is great important to clinical practice.When we have a whole understanding to the theory of stagnation,it is easy to analyze the obstruction-removing therapy.Though this principle hasn't clearly definition,it is connected closely with other regular treating methods.Based on it,the dissertation analyzes all kinds of obstruction-removing therapy in Shanghanlun.That will be very helpful for readers to understand this theory and its significance in clinical practice.
